Testosterone Therapy Explained
What Is Low Testosterone?
Testosterone plays a central role in how men feel, perform, and function every day. When levels drop below optimal range, the effects show up across nearly every area of life. Low libido, mood changes, loss of muscle mass, brain fog, and unexplained weight gain are among the most common signs that testosterone levels may need attention. For many men, these changes come on gradually and are written off as a normal part of aging. They are not something you simply have to accept.
At Optimized Wellness in Centennial, CO, we offer testosterone replacement therapy, or TRT, for men throughout the Denver South Metro area who are experiencing the effects of low testosterone and are ready to do something about it. Every treatment plan we build starts with a thorough lab evaluation so we have a clear, objective picture of where your levels stand before we recommend anything.
UNDERSTANDING LOW TESTOSTERONE
What Causes Low Testosterone
Testosterone levels naturally decline with age, typically starting in a man’s thirties, dropping by roughly 1% per year. But age isn’t the whole story. Chronic stress, poor sleep, excess body fat, certain medications, and untreated conditions like sleep apnea or type 2 diabetes can all accelerate the decline or cause low levels well before you’d expect them.
There’s no single “normal” number that applies to everyone. What matters is where your levels sit relative to your symptoms, your age, and your health history, which is why we don’t treat a lab result in isolation. We look at the full picture, including free and total testosterone along with related markers like estradiol and SHBG, to understand what’s actually going on before recommending anything.
How We Treat It
At Optimized Wellness, TRT is most often delivered through hormone pellets, small implants placed just beneath the skin that release a steady dose over several months. This avoids the peaks and crashes that can come with weekly injections and means fewer trips to the clinic for maintenance.
Every protocol includes regular follow-up labs, not just to confirm your testosterone is responding, but to monitor markers like hematocrit and PSA that matter for long-term safety. If something needs adjusting, whether it’s your dose or your monitoring schedule, we catch it early instead of waiting for a symptom to tell you something’s off.

“I have just begun going to ‘Optimized Wellness.’ Each person I have had contact with has been helpful, knowledgeable, and friendly. I am continuing to be a client at ‘Optimized Wellness.’ I felt taken care of.”

“Several months ago I came to Optimized Wellness and met with Amy because my psychiatrist recommended that I try bioidentical hormone replacement therapy. I had been struggling with what I thought was severe mental health issues for the previous year and a half and I was not responding to antidepressants that my primary doctor had prescribed me. In fact the antidepressants were making me very sick, and in the…”

FAQs
How do I know if I have low testosterone?
The only way to know for sure is a blood test, ideally taken in the morning when testosterone levels are naturally at their highest. Symptoms like low energy, reduced libido, and difficulty building muscle are worth paying attention to, but they overlap with a lot of other conditions, so labs are what actually confirm what’s going on.
Is TRT safe long-term?
When it’s monitored properly, yes. The risks people usually worry about, like elevated red blood cell counts or prostate concerns, are exactly why we run regular follow-up labs rather than starting treatment and hoping for the best. Your provider adjusts your protocol based on how your body responds, not on a fixed schedule.
How is TRT delivered: pellets, injections, or something else?
We primarily use hormone pellets, inserted just beneath the skin, that release testosterone steadily over several months. Most men find this more convenient than weekly injections, since it means fewer appointments and none of the highs and lows that can come with a shot-based schedule.
Will TRT affect my fertility?
It can. Testosterone therapy can suppress natural sperm production in some men, so if you’re planning to have children, this is something to bring up during your consultation. There are ways to manage this, and your provider can walk you through the options based on your goals.
How long before I notice a difference?
Most men start noticing changes in energy, mood, and mental clarity within the first few weeks. Physical changes, like muscle mass and body composition, tend to build more gradually over the following months as your levels stabilize.
